LASIK Surgery Questions, Lotemax
A letter to a new patient from Dr. Emil Chynn of Park Avenue Laser, the finest LASIK surgeon in New York City.
Dear Dr. Chynn,I saw Philip White and he appears to be doing well. The eyes are a little dry so he will continue with the Restasis for now.
Unaided vision is 20/70 both eyes
Refraction is RE:-1.50/+0.50×105=20/15-1
LE:-3.00/+0.75×70=20/20
Near vision:RE=J7 LE=J1
There is very slight corneal haze still IOP was 14mmHg in both eyes, so no problems with lotemax and pressure spikes. Overall he looks a bit more myopic than planned at this stage, but, as you say that should regress. Would you prefer to stop the lotemax all together? I enclose the topography as well. If you need anything more please let me know.

David,Thanks for your very good letter and care of our mutual patient.
As you said, he is slighly overcorrected in both eyes, but will regress. Your note didn’t say, but I assume he is on Lotemax 2x/day in both eyes. Because his initial Rx was low, I agree that he should stop his Lotemax completely now. He then needs to see you again in 3 weeks. At that time, off steroids, I expect both eyes to be less nearsighted.
His right eye should approach plano (0 Rx), and his left eye -2.00 (he wanted a lot of mono). Please email me his results on his return visit to you just after Valentine’s Day.
If he has not regressed sufficiently by then, I would start Voltaren 3x/day in both eyes.
This should regress him sufficiently after a month or two. Thanks for your help, and talk soon!

You probably can figure out what is going on based on the above email to your doctor. You are a bit nearsighted in both eyes, but that is planned/expected, as you will regress. If we didn’t do this, you would wind up farsighted again as you regressed. You should notice a gradual improvement in your distance vision off the Lotemax.
You should also notice that you can hold things a bit further to read them. Currently, your near vision is probably “super duper” but we actually don’t want it so good. So you should notice that now you can read microscopic print if you hold things very close.But that is not really our goal for your near vision.
We want you to read normal sized print at a normal distance.
So, as you regress, you will see that you can (and have to) hold things a bit further. After David sees you again in 3 weeks, if you have not regressed sufficiently. He will put you on a non-steroidal drop (NSAID) to regress you further. But this will also take some time (1-2 months) to work, so please be patient. Your reading should be good now, and you should be legal to drive in the daytime. I would not advise driving at night yet, until you regress further. Please email me your understanding of the above, and any further questions you may have.
