A patient returns home to Kyrgyzstan

My Dear Doctors!

Now I’m in my home.

As I have promised before I visited my ophthalmologist and retina specialist.

Both were interested and made all the procedures in order to check the results and to see the conditions of my eyes.

She informed that I have to visit once a month.

 That is what they ask me to send you:

VOD=0,7-0,8

VOS=0,5

OD sph +1,25-1,25×17

OS sph +0,5

Also, she wants to know about my minimum residual thickness of eye cornea after surgery…

 Everything is ok with me, now I’m working….

Every day 3 times a day I put my drops (Predforte) on, and I will continue till 15t of October and then start Lottamax

Also I take my vitamin C 2 times a day….

And of course thera tears …

One thing that disappoints me is blurring, I can’t see clearly, it is difficult to write an email or to read some information…

What will you suggest me to do?

Thank you in advance for your participation…

Looking forward for your answer

Best wishes,

Altynai S.

Dear Altynai:

            We are very pleased that you are doing well and also that you are taking care of your eyes keeping your follow up appointments with both, the Retina Specialist and your Ophthalmologist.

            The vision in each one of your eyes is great for your high prescription. It is actually better than what we tested you before your surgery. You are seeing better out of your right eye than your best corrected preop. Congratulations!!!!

            Remember, your vision is still going to improve during the following 2 months.

            Now, your reading up close vision is not that good right now, since you are intentionally overcorrected to allow a little bit of regression that always happen when treating any prescription and is more noticeable with extreme prescriptions like yours. We also adjusted your prescription so it will allow room for the expected growth according to your age. Once we switch your medications, your up close (reading) vision is going to improve. There is nothing for you to worry.

Starting now, please use your Lotemax (Mild steroid) three times a day for 1 month in both eyes (I want to taper that steroid slower, so we retain your positive prescription a little bit. You are going to start your Restasis too. That is once you wake up and once you go to bed. DO NOT DISCARD THE VIAL after each use, as directed by the manufacturer, USE UP EACH INDIVIDUAL VIAL since it is very expensive.

Please do not switch your medications next month to twice a day until you see your doctor there to make sure your corneas are clear and healthy.

            Your Residual corneal thickness is OD= 430 microns

                                                                    OS=413 microns

 In Summary:

Start Lotemax three times a day in both eyes.

Start Restasis twice a day in both eyes.

Keep using preservative free tears every hour and gel at night

Wear sunglasses when outdoor daytime and keep taking vitamin C 1000mg/day

Your reading up close vision is going to improve in the next few days after you switch steroids.

            Please keep me posted in your improvement of your reading up close vision, and feel free to ask any doubts/questions that you might have.

Sincerely,

Elias Almeida, MD

Refractive Fellow
