Introduction to VISX CustomVue and Advantages over Alcon and Bausch & Lomb lasers:
This clip is an introduction to the VISX CustomVue WaveFront system. It explains one important advantage that VISX CustomVue has over the Alcon and Bausch & Lomb systems--the ability to capture WaveFront images from an undilated pupil.
While this has time-saving advantages for both patient and doctor, there are additional, more important advantages.
"Part of the reason I chose the VISX CustomVue laser over the Alcon or Bausch & Lomb platforms is for efficiency--both my patients and my staff like the fact that we don't have to dilate people to capture the WaveFront on the VISX system," says Dr. Chynn.
"But more importantly, there are strong theoretical and practical reasons to prefer a system that captures the WaveFront image in its natural night-adapted state--without resorting to pharmacologic dilation with drugs. You see at night through a slightly dilated pupil, not a blown pupil due to phenylephrine. Therefore, many experts feel that this is a more accurate representation of how you actually see--which is what we want to capture and ultimately treat," Dr. Chynn explains.
"Take Joanna, who is my Laser Vision Coordinator. She is relatively young, and has large pupils. Her prescription going in was about a -4.00, with less than -1.00 of astigmatism. Amber, my Laser Technician, captured her WaveScan on the VISX platform, and adjusted her ablation diameter upwards to cover her entrance pupil. How is she seeing now? Let's ask her:
"Before my procedure, I was worried about my night vision, because I knew that I had large pupils," says Joanna. "I even went 'mystery-shopping' at other LASIK centers here in NYC, and was told at one place that I might not be a great candidate, because of my pupil size (almost 8mm). But Dr. Chynn explained to me that he would be performing a No-Flap LASEK on me, instead of a standard LASIK, and that this, along with maximizing the laser treatment area, would help reduce my chance of night glare."
"Now, 1 month after my surgery, my uncorrected vision with both eyes open is 20/10, or twice as good as 20/20, and much better than I ever saw in my contacts--without any night glare, whatsoever. In fact, my night vision now after my CustomVue LASEK is better than it was before, not worse!"

"WaveFront was developed by NASA to treat the imperfections in the Hubble Space Telescope," Dr. Chynn explains. Basically, many points of light are shone into your eye, pass through your entire optical pathway, and are reflected back, captured, and the deviations from perfection mapped out. People say the result is like a fingerprint of your eye, since no two people have identical WaveFront maps--even the two eyes are always different!
"This results in better vision than with standard, non-WaveFront treatment, which puts a uniform correction across your entire cornea, even though we know that nobody's visual pathway is perfectly uniform."
"I was the 1st surgeon in NYC to routinely combine WaveFront with No-Flap LASEK. This makes sense, as the WaveFront map is captured and saved on the computer before any flap is made, and making a flap causes higher-order aberrations, so capturing the treatment flap before inducing additional aberrations by making a flap is nonsensical!" Dr. Chynn exclaims. "In my hands, over 50% of my patients who elect to have WaveFront + LASEK are seeing better than 20/20 afterwards--remarkable!"
"This video clip you are about to watch explains how VISX has pioneered the use of Fourier analysis to reproduce the WaveFront map. Many studies have shown that the Fourier wave transformation can more accurately reproduce visual aberrations more accurately than Zernicke polynomials, at least at the order of polynomial commonly used. This is why VISX switched from Zernicke to Fourier in 2005--and parkavenuelaser.com did, too!
"We at parkavenuelaser.com are spending over $100,000 per year to constantly update all of our equipment to provide our patients with the best available technology. Iris Registration: Right out of James Bond and used to treat your eyes!
"Iris registration is just the latest advance from VISX," Dr. Chynn explains. "When patients are placed in the lying-down position under the laser, a minority of patients experience a significant cyclotorsional movement (rotation) of their eyes. No other laser platform yet in the US can compensate for this movement.
"Iris registration captures the unique peaks and valleys (iris crypts) in a patient's iris, and tracks to make sure no significant cyclotorsional movement occurs. If there is a cyclotorsional movement outside of acceptable parameters, the VISX laser will not allow you to fire.
"The system is so sensitive and accurate that it even identifies each eye of each individual patient. For example, if you try to treat the wrong patient or wrong eye, with iris registration, the laser will not let you proceed, because it 'knows' that you are trying to treat an incorrect eye! This is an additional safeguard that has never been possible before," Dr. Chynn says.
"Every patient is a bit nervous before their procedure--I was before my own LASIK in 1999, Missy, my Front Desk Manager, was before her LASEK in 2002, Amber, my Laser Technician was before her CustomVue LASEK in 2003, and Joanna, my Laser Vision Coordinator, was before her CustomVue LASEK in 2005. Guess what--I'm 20/20, and all of them are 20/10!
